What do I do if I have a prior approval and my healthcare service date changes?

If you have UltraCare or UltraCare 400 you don’t need to see an Affiliated Provider to be eligible for cover. 

On all other plans, some healthcare services are only covered if they’re performed by Affiliated Provider. If there is no Affiliated Provider in your area for your healthcare service, please contact us so we can discuss what you can do.
