What do I need to provide when submitting a claim?

We need a copy of your itemised receipts or invoices. These need to include the date your healthcare service took place, the name of the provider and the name of the person being treated. 

If you’re taking a photo of your receipt for online claiming, please make sure the image is clear and easy to read and all the information we need is visible. We can’t accept EFTPOS or credit card receipts.

The quickest way to claim is through the app or My Southern Cross. We’ll also process your claim quicker, within 5 days (paper claims can take longer). 

If you still want to claim using the paper form you need to send your itemised receipts or invoices (we recommend you keep a copy). These need to include the date your healthcare service took place, the name of the provider and the name of the person being treated. The policyholder also needs to sign the form. We can’t accept EFTPOS or credit card receipts.
